All the Active Ingredient Drugs
Imidazole Derivative Antifungal. Ketoconazole 200 mg. TABS.:60. Initiat. dose in adult.& adolesc. is 400-600 mg/d. taken orally in two or three divid. doses and this dose can be incr. rapidly to 800-1,200 mg/d in two or three divid. doses.
At tmt. initiat., 24-h urinary free cortisol should be controlled every few days/wks.
Adjust. of the posology
The dly. dose should be periodic. adjust. on an individ. basis with the aim to normalise urin. free cortisol and/or plasma cortisol levels.
A dose incr.of 200 mg/d. every 7- 28 d. may be consid. if urin. free cortisol and/or plasma cortisol levels are above the normal range, as long as the dose is tolerated by the pt.;
A mainten. dose from 400 mg/d. to a max. dose of 1,200 mg/d. taken orally in 2 to 3 divided doses may be requir. to restore normal cortisol levels. the maint. dose varied betwn. 600 mg/d. and 800 mg/d. See lit.
Indicated for the tmt. of endogenous Cushing’s syndr. in adult.& adolesc. above the age of 12 yrs.
C/I: Hypersens.
Hypersens. to any imidazole antifung. medication produc.
Acute or chron. hep. dis. and/or if pre-tmt. liver enzymes levels are above 2 times the upper limit of normal.

Lact.
Pts. with documented acquired QTc prolong.
Concom. ther. simvastatin, atorvastatin, lovastatin, eplerenone, methadone, disopyramide, quinidine, dronedarone, pimozide, sertindole, saquinavir (saquinavir/ritonavir 1000/100 mg bid), ranolazine, mizolastine, halofantrine, dabigatran, triazolam, oral midazolam, alprazolam.
Concom. ther. with ergot alkaloids (e.g. dihydroergotamine, ergometrine (ergonovine), ergotamine and methylergometrine (methylergonovine).
Concomitant therapy with lurasidone, quetiapine, telithromycin clarithromycin, felodipine, nisoldipine, colchicine, irinotecan, everolimus, sirolimus.
Concom. ther. with vardenafil in men older than 75-yrs.
Concom. ther. with paritaprevir/ombitasvir (ritonavir).
Concom. ther, with fesoterodine and solifenacin in pts. with renal impair.
Concom. ther. with tolvaptan.
Antifungal. Ketoconazole 20 mg/g. SHAMPOO: 120 ml. Use 2 x wkly for 2-4
wks. Wash and lather affect. skin, wait
5 mins. bef rinsing.
Top. tmt. seborrh. dermatit. incl. dandruff,
from fung. infect., pityriasis versicolor.

For adult use. To be applied on the affect. area once or twice/d for 2-6 weeks depend. on the type of infection. See prescrib. det.
For topical tmt. of dermatophyte infect. of the skin such as tinea corporis, tinea cruris, tinea manus and tinea pedis infections due to Trichophyton spp, Microsporon spp and Epidermophyton spp. Also for the tmt. of cutan. candidosis (including vulvitis), tinea (pityriasis) versicolor and seborrhoeic dermatitis caused by Malassezia (previously called Pityrosporum) spp.
