Ped. pts. 6 to 12 years of age: start 18 mg once/d, range: 18 mg to 54 mg once/d. Pts 13 to 17 years of age: start 18 mg once/d, range: 18 mg to 72 mg once/d (not to exceed 2 mg/kg/d). Adults 18 to 65 years of age: start 18 or 36 mg once/d, range: 18 mg to 72 mg once/d.
