VIAL ( pwdr. for sol. for infus.): 1, 5, 10×15, 50ml (2g/0.25 g). 1, 5, 10× 50ml (4g/0.5 g). Adult., adolesc.> 12 yrs. 4 g piperacillin / 0.5 g tazobactam given every 8 hrs. For nosoc. pneum., bact. infec. in neutr. pts., the recom. dose is 4 g / 0.5 admin. every 6 hrs. Nosoc. Pneum. Init. presum. tmt. of pts. with nosoc. pneum. start. dose 4 / 0.5 g every six hours plus an aminoglycoside, totaling 18.0 g (16.0 g piperacillin/2.0 g tazobactam). See lit.

C/I: Hist. of allerg. reac. to any of the penicillins, cephalosporins, β-lactam act. subst., (e.g. monobactam or carbapenem) or β-lactamase inhib. Hypersens. to the act. subst. or to any of IV solvent.
Other Cephalosporins & Penems. Ceftolozane 1 g, Tazobactam 0.5 g. VIAL (Pwdr. for concentrate for sol. for infus.): 10. The recomm. IV dose regimen for pts. with CrCl>50 mL/min.
Complicated intra-abdom. infec.*: 1 g ceftolozane/0.5 g tazobactam (To be used in comb. with metronidazole when anaerobic pathogens are suspected.) every 8 hrs., Infus. time 1 h, duration of the tmt. 4-14 days.
Complicated UTI's, acute pyelonephritis: 1 g ceftolozane/0.5 g tazobactam, every 8 hrs., infus. time 1h, 7 days.
