Dosage
Adults and adolescents above 12 years of age: The following table is a guide to dosage for the more commonly used blocks. The smallest dose required to produce an effective block should be used.The clinician’s experience and
knowledge of the patient’s physical status are of importance when deciding the dose.

Indications
Surgical anesthesia: Epidural block for surgery, including Caesarean secion, major nerve block, field block. Acute pain management: Continuous epidural infusion or intermittent bolus administration, e.g postoperative or labor pain, field block.
Contra-Indications
Hypersensitivity to local anesthetics of the amide type. General contraindications related to epidural anesthesia, regardless of the local anesthetic used. I.V. regional anesthesia. Obstetric para cervical anesthesia. Hypovolemnia.
Special Precautions
Certain local anesthetic procedures such as injections in the head and neck regions may be associated with a higher frequency of serious adverse reactions, regardless of the local anesthetic used. Attempts should be made to optimize the patient’s condition before major blocks are performed. In patients with severe liver disease, repeated doses may need to be reduced due to delayed elimination.

Side Effects
Very common: Hypotension, nausea.
Common: Paresthesia, dizziness, headache, bradycardia, tachycardia, hypertension. Vomiting, urinary retention, temperature elevation, rigor, back pain. Predominantly gastrointestinal: Diarrhea, abdominal pain, nausea, constipation, flatulence, cholelithiasis, gall bladder sludge.
Drug interactions
CNS depressants.
